Glaucoma Treatment

For some patients the effectiveness of medications and eye drops may lessen over time, while for others there are unpleasant side effects. Because glaucoma often is painless, many patients become careless about the strict use of their medications. In fact, non-compliance with a program of prescribed glaucoma medications is a major reason for blindness caused by the disease.


Glaucoma Surgery

Sometimes medications aren’t effective and surgery becomes necessary. Rosenbaum Eye and Laser Center offers different surgery options to best fit your needs.

iStent

The iStent is a procedure to help safely and effectively manage your eye pressure. The iStent is used in conjunction with cataract surgery to help reduce the intraocular pressure in patients with open-angle glaucoma. The iStent creates a permanent opening in the eye’s natural drainage system to improve the eye’s natural fluid outflow. By improving the fluid outflow, the intraocular pressure lowers. You may be able to experience a reduction in your glaucoma medications after having the iStent procedure.

Laser Peripheral Iridotomy (LPI)

Laser Peripheral Iridotomy (LPI) is type of laser procedure used in closed-angle glaucoma.  It is used to equalize the pressure between the front and back chambers of the eye.  This is done by using a laser to create a very small hole(s) in the iris to allow the fluid to drain from the back to the front of the eye where it can be drained through the trabecular meshwork. The LPI can be done prophylactically (preventively), or on an emergent basis if there is a sudden increase in eye pressure.

SLT Laser

Selective Laser Trabeculoplasty (SLT) is a highly effective laser procedure that reduces the pressure inside the eye.  It uses short pulses of low-energy light to target cells in the drainage system to increase drainage and help lower the eye pressure.  Given the gentle action of SLT, it can be offered as an option for primary therapy in place of starting topical medications with less concern about side effects and patient compliance.  It is performed in the office and takes approximately 5 minutes.  There is a reported 93% success rate over 5 years with SLT as primary therapy.  You do still need routine follow-up to monitor your glaucoma with your eye care provider.  Give our office a call for a consultation to see if a SLT treatment is on option for you.

Trabeculectomy (Filtering Microsurgery)

Trabeculectomy (Filtering Microsurgery) is usually recommended only if eye drops and laser eye surgery cannot lower the eye pressure to a safe level.  In this procedure, a new drainage canal (filter) is created surgically for the eye. The excess fluid in the eye is then collected in this filter and drained into a tissue pocket (bleb) under the upper eyelid. The fluid is then absorbed by tiny blood vessels and returned into the body’s circulation. The trabeculectomy is usually performed in an out-patient setting at a hospital or surgical center.

Tube Shunt Surgery

Tube Shunt Surgery is a surgical procedure in which a glaucoma drainage device, or implant, is actually inserted into the eye to bypass the obstructed drainage system of the eye.  It is usually only recommended for very severe cases of glaucoma.
